24-25 Mid-Season Coach Evaluation

The purpose of this Coach Evaluation is to allow parents and players* the opportunity to provide feedback on the coaching staff for the first half of the season. All submissions are confidential, however please note that feedback will be provided to coaches at the discretion of the Board. While this feedback is primarily used as part of the coaching selection process, we may use the feedback as opportunities for current coaches to make adjustments in their practices on their current teams. Please feel free to provide an evaluation for the Head Coach and any Assistant Coaches on your team. We thank-you for your input. *We strongly encourage U15 and U18 aged players to submit their own coaching evaluations. Parents are welcome to submit a separate evaluation.
